Job summary

Street Culture Project in Regina, SK is seeking a Case Plan Coordinator to oversee intake, case management, and transition planning for youth in Transitional Housing programs. This role requires strong organizational skills and a commitment to trauma-informed, youth-centered care.

Work hours are 8:30 am to 5:00 pm, Monday to Friday, with a full-time position and compensation in the $21.50–$24.00 per hour range.

Qualifications

• 2+ years of experience in youth care or case management.
• Experience in case planning, advocacy, or resource coordination.
• Strong communication, documentation, and organizational skills.
• Understanding of trauma-informed care and harm reduction.
• Ability to work collaboratively and independently in a fast-paced environment.
• Excellent communication skills and the ability to remain calm and decisive under pressure.

Responsibilities

• Manage the Journey: Lead the referral, intake, and discharge process, ensuring all transitions are smooth, documented, and youth-centered.
• Build the Roadmap: Create and finalize individualized case plans within strict timelines, coordinating with the youth’s full "circle of care" (MSS, medical, and education).
• Advocate for Access: Identify service gaps and connect youth to community resources, health supports, and educational opportunities.
• Master the Details: Maintain 100% accurate records in systems like Apricot, ensuring safety plans, MARs, and case notes are updated the same shift they occur.
• Lead in Crisis: Develop youth-specific safety plans and provide support and clarity during high-stress situations.

Street Culture Project is hiring a Case Plan Coordinator (CPC) to oversee the intake, case management, and transition planning for youth in our Transitional Housing programs.

What You’ll Do

You will be the clinical architect for our youth—managing referrals, building individualized case plans, and ensuring our documentation meets the highest standards of excellence. You’ll:

• Manage the Journey: Lead the referral, intake, and discharge process, ensuring all transitions are smooth, documented, and youth-centered.

What You Bring

We’re looking for someone with:

• 2+ years of experience in youth care or case management (required)
• Experience in case planning, advocacy, or resource coordination (preferred)
• Strong communication, documentation, and organizational skills
• Understanding of trauma-informed care and harm reduction
• Ability to work collaboratively and independently in a fast-paced environment
• Excellent communication skills and the ability to remain calm and decisive under pressure

Additional Requirements

• Secondary school completion (post-secondary preferred)
• Criminal Record Check with Vulnerable Sector clearance
• Valid SK Driver’s License
• Knowledge of SCP's values and approaches (training provided)
• Strong time management and documentation skills
• Training in CPR/First Aid, ASIST, Crisis Intervention, or Mental Health First Aid is an asset
