The Case Manager Employment Consultant plays a key role in supporting clients to secure and maintain meaningful employment by creating tailored action plans and providing continuous guidance. This position focuses on building strong relationships with clients and community partners, advocating for job opportunities, and addressing employment barriers. The role requires proactive engagement in tracking client progress, managing case files, and ensuring alignment with YMCA and funder standards. A results-driven approach is essential to meet program goals and drive the success of employment initiatives.

In this role, you will:

  • Develop strategies and timelines for individual clients to achieve goals and solve obstacles or challenges with respect to sustainable employment.
  • Ensure self-management of program targets related to employment program delivery and funder expectations. Complete and maintain client files as per funder and YMCA quality standards.
  • Follow up with individual clients to track 1, 3, 6, and 12-month outcomes, ensuring proper documentation of proof of employment outcomes.
  • Develop strategies and timelines for individual clients to achieve goals and problem-solve when obstacles or challenges occur.
  • Record and report services provided using an electronic tracking system. Maintain and update client information and outcomes in the database.
  • Advocate for clients with community employers, services, and programs to advance their employment and personal goals. Collaborate with team members and community partners to identify job opportunities, track client progress, and participate in outreach events to build and maintain community relationships.
  • Connect clients with necessary services and resources. Broker support where relevant and establish or strengthen partnerships with service agencies through targeted community outreach.
  • Provide advocacy support to clients with personal, systemic, and/or environmental barriers (e.g., addictions, chronic disease, homelessness, mental health).
  • Use the Common Assessment tool to develop an appropriate Employment Action Plan with clients based on assessment.
  • Manage case work by conducting needs and risk assessments, developing contracts, implementing plans, and following up on goals and other needs. Use a standardized intake process to assess individuals’ Employment Service and Life Stabilization needs and identify requirements for job and life skills coaching.
  • Contribute to monthly individual and team Key Performance Indicators (KPIs). Manage tasks through prioritization and effective time management.
  • Prepare case management and case note documentation. Handle confidential data and ensure quality audit measures are achieved in accordance with YMCA and funder standards.
  • Perform other duties as assigned.

You bring:

  • Post-secondary education in Career Planning, Employment Counselling, Social/Community Work, Social Services, Human Resources, or a related field, or equivalent experience.
  • Minimum two years of experience in counselling, file management, group facilitation, or job coaching is preferred.
  • Proficiency with personal computer applications, including Microsoft Office, Microsoft Outlook, and Internet search engines.
  • Knowledge of issues that prevent individuals from being employed and the ability to work with clients in a respectful, non-judgmental, and consistent manner.
  • Knowledge of the philosophy of Harm Reduction, substance use, mental health, and/or concurrent concerns.
  • Knowledge of community resources, familiarity with the labour market, as well as apprenticeship and training opportunities.
  • Ability to develop and maintain strong professional relationships with employers, businesses, community partners, and colleagues for the purpose of coordinating case management.
  • Ability to provide excellent service delivery.
  • Reliable access to transportation, as extensive travel within the region is required.
  • Well-developed interpersonal and relationship-building skills; ability to establish rapport and communicate effectively with clients, staff, and volunteers.
  • Excellent presentation and written communication skills.
  • Ability to work flexible hours, including days, evenings, and weekends.
  • Excellent understanding of diversity, inclusion, and equity, including the ability to identify and address barriers to employment, work effectively with diverse stakeholders, and integrate equity into program design.
  • Focus on the health, safety and wellbeing of all children, youth, and vulnerable people in YMCA programs including understanding legal and moral obligations related to reporting suspected child abuse to Child Protection Services
