Job summary

VMCH is seeking a Care Partner to coordinate person-centred home-based care for older adults across Eastern and Southern Metropolitan Melbourne. You will build meaningful relationships with clients, manage budgets and develop care plans, with regular travel to regional areas.

The role emphasises independence, quality standards, and a supportive multidisciplinary team. A Diploma or higher in a relevant field is required, with Victorian Driver’s Licence and Police Check.

Qualifications

  • Diploma qualification in a relevant field or higher.
  • Experience in case management, care coordination or client services within aged care or community services.
  • Experience completing assessments and developing care plans for clients with complex needs.
  • Ability to manage consumer budgets and support services effectively.
  • Sound computer skills (Microsoft Office and client management systems).
  • Excellent organisational skills with the ability to prioritise and work autonomously.
  • Understanding of community aged care services, referral pathways and person-centred care principles.
  • Knowledge of aged care legislation, quality standards and sector reforms.
  • Current Victorian Driver's Licence and car registration.
  • Willingness to travel across Metropolitan & Regional areas.
  • National Police Check.

Responsibilities

  • Deliver high-quality, person-centred care in clients' homes and communities.
  • Conduct comprehensive assessments, develop care plans and coordinate services.
  • Empower clients and carers to make informed choices about care and support.
  • Manage your client portfolio, including budgets, services and changing care needs.
  • Ensure services align with aged care quality standards and policies.
  • Maintain accurate case notes and client confidentiality.
  • Collaborate within a multidisciplinary team and escalate concerns when needed.
  • Work towards KPIs and service delivery targets while balancing quality and productivity.

Location: Pakenham, VIC, Australia 3810. Contract: Full-time.

As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities.

Based in Pakenham, you’ll support clients across Eastern and Southern Metropolitan Melbourne, extending through to Gippsland. Regular travel is an important part of the role, allowing you to build meaningful relationships with clients and coordinate personalised services that enhance their wellbeing, independence and quality of life.

In This Role, You Will
  • Coordinate person-centred services within the Support at Home environment, supporting clients to achieve their goals and remain independent at home
  • Deliver high-quality, person-centred care and support to clients in their homes and communities
  • Conduct comprehensive assessments, develop individualised care plans and coordinate services
  • Empower clients and carers to make informed choices about their care and support
  • Manage your own client portfolio, including budgets, services and changing care needs
  • Ensure services are delivered in line with aged care quality standards and organisational policies
  • Maintain accurate case notes, documentation and client confidentiality
  • Work collaboratively within a supportive, multidisciplinary team and escalator concerns where required
  • Work towards agreed KPIs and service delivery targets, balancing quality client outcomes with productivity and service growth.

What’s Required
  • Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
  • Experience in case management, care coordination or client services within aged care, community services or a similar environment
  • Experience completing assessments, developing care plans and supporting clients with complex needs
