Ontario Health atHome is seeking a Care Coordinator (RN, PT, OT or SW) for Sault Ste. Marie to assess patient needs, determine eligibility for services and coordinate care plans with families and providers. This full-time role emphasizes patient-centered care and seamless community connections.

Qualifications

  • Membership, in good standing, with the applicable regulatory body.
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • Recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
  • Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s license and access to a reliable vehicle
  • Proficient in a Windows environment
  • Advanced oral and written proficiency in English is essential
  • COVID-19 vaccination policy compliance

Responsibilities

As a Care Coordinator (Full-Time, Start Date: Immediately Location: Sault Ste. Marie – Located in Sault Area Hospital ), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners .

  • Case management experience or recent related community experience
  • Ability to speak French or another second language

Salary: $41.29 to $51.13 as per the ONA Collective Agreem
