Ontario Health atHome seeks a Care Coordinator (RN, PT, OT or SW) for a full-time role starting immediately in North Bay or Kirkland Lake. You will assess patient needs, determine eligibility for services, and connect patients with community resources and care teams.

You will work in a collaborative, multidisciplinary setting to support patients and families, ensuring accessible, patient-centered care at home and a strong emphasis on continuity of care.

Qualifications

  • Registration in good standing with regulatory bodies: CNO, CPO, OCOT, OC SW.
  • Recent experience in community health or related field.
  • Knowledge of health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Time management, prioritization and organization; ability to work independently in a busy multidisciplinary environment.
  • Valid driver’s license and access to a reliable vehicle.
  • Advanced English and French proficiency are essential.
  • COVID-19 vaccination status submission as a condition of employment.

Responsibilities

  • Assess care needs, determine eligibility for services, and develop individual care and service plans.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish a helping relationship with patients and their families.
  • Balance patient needs with available resources, respecting values and preferences.
  • Initial assignment; possible changes based on operational needs.

As a Care Coordinator (Full-Time, Startdate: Immediately; Location: North Bay or Kirkland Lake), you willassess and determine patient care needs and eligibility, provide access andreferrals to community services, and engage with patients, caregivers and otherhealth care practitioners.

What must you have?

  • Membership,in good standing, with the applicable regulatory body:
  • College ofNurses of Ontario
  • Collegeof Physiotherapists of Ontario
  • Collegeof Occupational Therapists of Ontario
  • OntarioCollege of Social Workers and Social Service Workers
  • Recentexperience in community health or a related field
  • Knowledgeof the health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • Avalid driver’s license and access to a reliable vehicle
  • Advancedoral and written proficiency in English and French is essential
  • Wehave a mandatory COVID-19 vaccination policy. As a condition ofemployment, all employees are required to submit proof of COVID-19vaccination status prior to start date.

Bilingual candidates with advancedoral and written proficiency in English and French will be given firstconsideration; however, less qualified candidates who don’t meet the languagerequirements may still be considered.

What would give you the edge?

  • Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
  • Casemanagement experience or recent related community experience
  • Abilityto speak French or another second language

French Proficiency: French an asset

Salary: Hourly wage of $41.29 to $51.13
