Ontario Health atHome in North Bay is seeking a Care Coordinator (RN/PT/OT/SW) to assess patient needs, determine eligibility for services, and link clients with community providers. The role supports patients to be healthier at home and collaborates with a multidisciplinary team.

Requirements include active registration, community health experience, strong communication and organizational skills, and a valid driver’s license. COVID-19 vaccination is required prior to start date.

Qualifications

  • Registration in good standing with the applicable regulatory body (Nurses Ontario, Physiotherapists Ontario, Occupational Therapists Ontario, Ontario College of Social Workers and Social Service Workers).
  • Recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment.
  • Established ability to accurately complete required documentation, reports and forms.
  • A valid driver’s license and access to a reliable vehicle.
  • Proficient in a Windows environment.
  • Advanced oral and written proficiency in English is essential.
  • We have a mandatory COVID-19 vaccination policy.

Responsibilities

  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans.

As a Care Coordinator (Full-Time, Start Date: Immediately, Location: North Bay Regional Health Centre ) , you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners .

What will you do?

What must you have?
  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • Recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
