Job summary

Ontario Health atHome seeks a Hospital Care Coordinator to assess patient needs, determine eligibility for services, connect patients with community providers, and coordinate delivery of care plans with families. Ideal candidates hold active registrations with Ontario regulatory bodies and have 2+ years in community health.

Qualifications

• Active registration with Ontario regulatory bodies (Nursing/Physiotherapy/Occupational Therapy/Social Work/Speech-Language Pathology) in good standing.
• 2+ years of recent experience in community health or related field.
• Knowledge of health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
• Effective time management, prioritization and organizational skills; ability to work independently and in multidisciplinary teams.
• Ability to accurately complete required documentation, reports and forms.
• Valid driver’s licence and access to a reliable vehicle.
• Proficient in a Windows environment.
• Mandatory COVID-19 vaccination status proof prior to start date.

Ontario Health atHome is seeking a qualified candidate to join the Milton Hospital Team in a full-time, permanent position.

This role involves an extended-hours schedule, consisting of 11‑hour shifts, inclusive of a one‑hour unpaid break, and requires working every other weekend.

What must you have?

• Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • College of Audiologists and Speech Language Pathologists of Ontario
• 2+ years of recent experience in community health or a related field
• Knowledge of the health care delivery system and community resources
• Excellent interpersonal, communication, assessment, problem‑solving, and decision‑making skills
• Effective time management, prioritization and organizational skills, with the ability to work independently and co‑operatively in a busy multidisciplinary environment
• Established ability to accurately complete required documentation, reports and forms
• A valid driver’s licence and access to a reliable vehicle
• Proficient in a Windows environment
• We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID‑19 vaccination status prior to start date.
