Job summary

Kaizen Lab Inc. is seeking a Youth Support Worker to build supportive relationships with youth and young adults within the Phoenix Center’s YAYA program.

You will work with a multidisciplinary team to help clients reach personal goals, ensure safety, and transition to healthy independent living. The role involves coordinating activities, responding to immediate needs, assisting with daily tasks, and applying trauma-informed practices.

Qualifications

  • Grade 12 or equivalent
  • Certificate in Community Social Service or equivalent
  • 1 year of relevant youth services experience
  • Valid Class 5 driver’s license with clean abstract
  • Standard First Aid with CPR-C/AED where available
  • Food Safe certification preferred
  • Nonviolent Crisis Intervention (NVCI) completion
  • Food Safe certification required

Responsibilities

  • Welcome and orient youth/young adults to Phoenix services; provide program orientation and tours
  • Engage with at-risk youth aged 17–24 and build trusting relationships
  • Plan and coordinate youth meetings, activities, and life skills sessions
  • Assist with immediate needs (food, clothing, recreation) and daily activities
  • Support clients with care plans and goals in partnership with MHSU team
  • Assist youth with accessing external supports and accompany as needed
  • Respond to medical or behavioral concerns using trauma-informed practices
  • Maintain residence cleanliness and safety; manage room searches on discharge
  • Manage youth case files and required reporting
  • Attend trainings and meetings as directed by supervisor
  • Ensure safety and security of the residence and client whereabouts
  • Complete incident reports per policy
  • Other duties as required

Job Summary:
The primary role of a Youth Support Worker is to buildtrusting, supportive professional relationships with youth andyoungadults. This role works with a multidisciplinary team to support clients tohelp them reach their goals andtransition to healthy, confident adults asper a pre-established plan of care. Working in the Youth & YoungAdult(YAYA) Program, this role will provide youth and young adults withtheir basic and immediate needs, ensure theirphysical and emotional well-beingand safety, support them with relapse prevention and treatment, and helpthemto transition back to the community when they may be ready.

Client Services:

  • Welcomeand orient youth/young adults accessing Phoenix services upon entry to theProgram (i.e., introduceand explain services provided, facilitytours, complete program orientation, etc.)
  • Activelyengage with and develop relationships with at-risk youth and young adults17-24 years of age
  • Coordinateyouth meetings and activities, including recreation and life skills,individually and in consultation withthe primary treatment providers
  • Respondto and assist with the provision of clients’ immediate needs, includingfood, clothing, recreationalopportunities, etc.
  • Assistclients with the completion of instrumental daily activities, includinghousehold upkeep and communityparticipation
  • Asper pre-established care plans, work collaboratively with other staff toassist youth in attaining short andlong-term goals, strategiesformeeting these goals, and an action plan for implementation, based oninputs provided by the MHSU team.
  • Assistyouth with accessing external community supports and accompany them ifneeded
  • Respondto any medical emergencies or potential behavioral, mental health,emotional, or substance useconcerns using pre-establishedtrauma-informed practice.
  • Maintainthe cleanliness and safety of the residence, including laundry asneeded
  • Manageyouth case files, including all relevant reporting and documentation
  • Attendall training and meetings as directed by your manager
  • Completeroom searches and room cleans upon client discharging from program
  • Monitorsite security and respond to safety concerns, including access to/from theresidence, keeping track ofclients’ whereabouts, etc.
  • Completecritical incident reports as per policy
  • Otherduties as required

Qualifications:

  • Grade 12
  • Certificate in Community Social Service orequivalent
  • Recent, related experience of one year providingcommunity social services to youth and young adults andworking withyouth and young adult populations, substance use services, mental health,corrections, etc., orequivalent
  • Valid Class 5 driver’s license with clean driver’sabstract
  • Standard First Aid with CPR-C-AED certificatepreferred
  • Food Safe certificate preferred
  • Satisfactory completion of Nonviolent crisis intervention (NVCI)
  • Food Safe Certification is required
