Job summary

ConnectFASD Full Time Calgary, AB Hybrid role focuses on advocacy and support for adults affected by FASD. You will work within the CARE Connector team to develop client-centered CARE plans and coordinate services across agencies.

As part of the role, you will advocate for clients, navigate complex systems, maintain records with privacy in mind, and travel within the greater Calgary region. The position is on-site with some community-based work and a 36-hour salaried structure.

Qualifications

• Diploma in Health, Social Services, or related field (or equivalent experience).
• Knowledge and understanding of FASD and its impact on individuals and communities.
• Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with clients and families.
• Compassionate, empathetic, and nonjudgmental approach.
• Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and priorities effectively.
• Ability to work independently and as part of a team, with a commitment to collaboration and cooperation.
• Proficiency in Microsoft Office Suite, project management software, and other relevant tools.
• Must have a driver’s licence, own vehicle, and $2 million in vehicle liability insurance.

ConnectFASD Full Time Calgary, AB Hybrid $53,932.86 - $57,767.55 CAD per year

The Coordinated Access, Response and Empowerment (C.A.R.E) Connector plays a vital role in advocacy and support to those impacted by Fetal Alcohol Spectrum Disorder (FASD). Working within a team of Connectors, the C.A.R.E Connector provides client-centred, comprehensive support across the lifespan for individuals, families, and communities impacted by FASD. This role supports primarily adults.

ACCOUNTABILITIES

Assessment and Planning

• In conjunction with the Intake Team, support the development of individualized C.A.R.E plans in collaboration with the client to meet their needs.
• Regularly review and update C.A.R.E plans to reflect progress and changing needs.

Case Management

• Provide ongoing support and guidance to clients as outlined in the client-directed C.A.R.E plan.
• Serve as a strong advocate for individuals with FASD to ensure their needs are understood and met within their communities.
• Assist clients in navigating complex systems and facilitate referrals to specialized services as needed.
• Coordinate services and support from multiple agencies and providers, ensuring continuity of care and effective collaboration.
• Monitor service delivery and follow-up to ensure that clients receive appropriate and timely interventions.

Documentation and Compliance

• Maintain accurate and up-to-date records of client care plans, progress notes, and service provision.
• Maintain strict adherence to privacy laws and ethical guidelines regarding the handling of sensitive client information, including obtaining all necessary consents for information sharing with other professionals/agencies as part of care coordination.
• Prepare written reports and documentation for internal purposes, funding agencies, and regulatory bodies as required.

Working Conditions

• Community-based work requiring access to reliable transportation (driver’s licence/vehicle/insurance required).
• The CARE Connector works a 36-hour fixed, salaried position of 4 days per week. The working location for this position is on-site in the office, in the community, and in client homes, with Mentors working a minimum of 2 days per rotation in the office.
• Occasional evening and weekend work is required. Travel is required within the greater Calgary region to support clients. Work is fast-paced, with multiple projects and objectives in motion and will require moderate physical activity. Some activities may require moving up to 40 lbs.
