Job summary

Southbridge Care Homes is seeking a caring Behavioral Support Nurse – RPN to join our team in Ottawa. The BSO – RPN will coordinate behavioral support for residents, apply dementia care knowledge, and collaborate with caregivers and medical staff to promote safety and well-being.

The ideal candidate will be an RPN with a current Ontario college certificate, at least two years in a similar role, and the ability to work both independently and as part of a team. Bilingualism is a plus.

Qualifications

  • Registered Practical Nurse with current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience in a similar role, preferably in the long-term care setting.
  • Degree in human psychology or behavioural sciences is an asset.
  • Bilingual in English and French is an asset but not required.
  • Ability to work independently and as part of a team with strong communication.

Responsibilities

  • Deliver person-centered care and promote residents' abilities and self-esteem.
  • Conduct assessments using observational tools to describe behaviors and risks.
  • Ensure care plans address responsive behaviors and are updated weekly.
  • Communicate with external stakeholders and management to identify risks.
  • Participate in quality improvement activities and provide monthly reports.
  • Provide coaching and mentorship to staff for collaborative care.
  • Conduct resident care reviews and attend quality program meetings.
  • Collaborate with pharmacists and physicians regarding medication effects.
  • Participate in staff development programs and ongoing training.
  • Maintain relationships with staff, residents, and the public; assist with admin duties.

Salary Range: $55,000.00 - $60,000.00

Southbridge Care Homes, our brand new facility, is looking for a caring, passionate, and enthusiastic Behavioral Support Nurse – RPN, to add to our amazing team. You will serve our residents, to keep them safe, healthy, and happy. Reporting to the Director of Care and Assistant Director of Care our BSO – RPN will contribute by sharing behavioral support to the residents, demonstrate knowledge with dementia care and mental health issues. An individual will be responsible for coordinating with multiple stakeholders for the development of a person-center person center philosophy of care for the residents.

As a BSO – RPN, you will be accountable for…
  • Contribute in delivering person-centered care by promoting the abilities and self esteem of the residents, develop strategies to promote optimal quality of life for the residents with the responsive behaviors.
  • Conduct assessments using various assessment tools which describes behaviors of residents including observational behaviors, identifying behavioral risks, and identify caregiver strategies.
  • Ensure the plan of care is regulated with responsive behaviors of residents in regards to the staff providing care and mitigating risks by conducting weekly rounds.
  • Communicate with the external stakeholder and management team to identify the risks associated with responsive behavior.
  • Demonstrate participation in quality improvement activities and provide monthly reports.
  • Provide coaching and mentorship to the staff to ensure collaboration of care and services for the responsive behavior.
  • Conduct resident care reviews and attend quality program meetings including P.A.C.
  • Collaborate with the pharmacists and physicians for the consultation regarding the affect of medication treatment on residents.
  • Participate in the staff development programs and seminars, and require to attend ongoing training to support skills in managing residents with responsive behaviors.
  • Assist in tasks by considering policies of the home and the behavioral support Ontario project.
  • Maintain amazing relationship with all the stakeholders including staff, residents and the general public.
  • Provide an excellent customer service to all the stakeholders and assist in the administrative duties of the home.
  • Other duties as required.
The ideal candidate will have the following qualifications and skills:
  • Registered Practical Nurse
  • Appropriate current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience required in the similar role, preferably in the long term care setting, with comprehensive knowledge of duties.
  • Degree in human psychology, or behavioral sciences would be considered as an asset
  • Bilingual in English and French is an asset but not required.
  • A serving heart and a passion for caring for seniors.
  • Registration/eligible for registration with the Registered Practical Nurses Association of Ontario.
  • Ability to work individually and as part of a team, to prioritize tasks in order of urgency.
  • Excellent communication (verbal and written) and comprehension skills.
  • Developed creative problem-solving skills to look at the “big picture.
