Job summary

Sault Area Hospital is seeking a Registered Practical Nurse to focus on transitions of care for seniors, including those in hospital waiting for Long Term Care. You will work under an RN-led team and partner with family, caregivers, and community LTC homes to ensure safe, smooth transitions.

You will use geriatric knowledge and evidence-based practices to support patients and families through discharge, with emphasis on reducing lengths of stay and ensuring continuity of care.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current registration with the College of Nurses of Ontario in good standing.
• Certification in a Geriatric Education program such as NICHE, P.I.E.C.E.S., Positive Approaches to Care or equivalent
• A valid driver’s license and full time use of reliable vehicle is required

Job description

The BSO RPN will focus on transitions of care for patients living in the community or currently in hospital waiting for transfer to Long Term Care. The RPN will work under the guidance of an RN-led team and collaborate with community partners, Long Term Care Homes, and family/caregivers. Utilizing best practice guidelines and specialized geriatric knowledge, the RPN will collaborate with partners to ensure a safe and successful transition to LTC for patients. It is critical that patients and their families feel supported during the transition to LTC.

Duties

• Effectively use the Standards of Nursing Practice and the nursing process to assess, plan, implement, and evaluate patient care of the senior with mental illness and/ or behavioral disturbances
• Assist in the development and implementation of standardized protocols to assist the individuals with responsive behaviours of those in the hospital setting
• Assist in the planning and implementation of recommendations to increase successful transitions to LTC
• Participates collaboratively with other health care providers in the provision of care
• Liaises with the psychiatrists, family physicians, LTC community and other care providers to ensure continuity of care and to reduce lengths of stay in the hospital.
• Works in collaboration with the BSO Integrated Response Team in the district of Algoma
• Participates in data collection for statistical, Ministry and research purposes

Qualifications

Experience

• Minimum of 3 years’ experience in assessing and treating adults/seniors with experiencing mental health, dementia, delirium, other neurological disorders;

Ability

• Demonstrated knowledge and evidence-based best practice in dementia, delirium, mental health issues, and their effects on persons, families, and caregivers;
• Knowledge of community support services, primary health care and mental health systems and related legislation;
• Knowledge of consent and capacity issues and legislation involving this population (i.e., Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act, Mental Health Act, Long-term Care Act, PHIPA);
• Ability to self-direct and collaborate within the context of an interdisciplinary team;
• Proven interpersonal, problem-solving skills to establish and maintain positive working relationships with persons with lived experience, family members, program staff, and agencies;
• Demonstrated excellent therapeutic communication and interviewing skills specific to mental health issues/illnesses.
• Strong client & family service orientation.
• Demonstrated patient teaching skills, for both patients and caregiver
• Strong organizational and time management skills, and the ability to take initiative and work autonomously;
• Knowledgeable in the utilization and care of all mechanical equipment required for patient care.
• Demonstrated ability to maintain successful working relationships; ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner, both in person and on the telephone; tact, courtesy and alertness in public contacts.
• Proficient in computer applications; with ability to utilize OTN (Ontario Telehealth Network) equipment
• Demonstrated behavior consistent with the hospital’s mission statement and core values.
