Job summary

Ontario Health atHome in Labelle, Ontario, seeks a qualified Bilingual Care Coordinator (Palliative Care) to assess patient needs, determine eligibility, and connect families with community resources. The role involves home visits or calls, developing care plans and coordinating with MAiD support as needed.

You will join a Palliative Team, work 8:30am–4:30pm, Monday to Friday, with 1.0 FTE and a starting salary of CAD$44.48–$47.52 per hour. Fluency in French and English is required.

Qualifications

• RN with BScN or nursing diploma and current registration.
• Palliative care experience including advance care planning.
• Ability to work bilingual in French and English.

Responsibilities

• Assess patient needs and determine eligibility for care.
• Develop and update individualized care plans.
• Coordinate referrals to hospice, MAiD assistance as needed.
• Facilitate home visits and monitor patient and caregiver needs.
