Job summary

Perennial is seeking a Bilingual Patient Care Coordinator to join our Ontario and Alberta teams. You will be the primary contact for residents and families, coordinating care plans and services with clinicians and operations staff.

The role emphasizes clear communication, empathy, and efficient scheduling to ensure timely, compassionate care across retirement residences. Fluency in French and English is required, with occasional travel to Ontario homes.

Qualifications

• 2+ years in a patient-facing or healthcare coordination role (clinic, hospital, retirement, or wellness setting strongly preferred).
• Experience coordinating appointments, referrals, follow-ups, or services across multiple stakeholders.
• Diploma or degree in healthcare administration, medical office administration, social services, or related field.
• Excellent organizational skills, ability to multitask, and strong written and verbal communication skills.

Responsibilities

• Act as the “go-to” contact for residents and families in Ontario and Alberta, responding promptly to calls, emails, and texts.
• Schedule and confirm appointments with nurse practitioners, physicians, and allied health providers; coordinate referrals and follow-ups.
• Liaise with primary care providers, nurse practitioners, and operations staff to ensure clear, timely communication about resident needs.
• Troubleshoot resident questions or concerns, escalating as needed.
• Guide residents and families through care plans, insurance submissions, and wellness program offerings.
• Partner with operations to ensure smooth day-to-day workflows across homes.
• Maintain accurate records in EMRs and internal systems.
• Foster trusted relationships with residents, families, and on-site staff.
