Job summary

Ontario Health atHome in Ottawa is seeking a Bilingual Care Coordinator with a strong nursing background to manage a palliative caseload. You will visit patients at home or connect via phone, assess needs, and develop care plans in alignment with patient and caregiver goals.

The role requires English and French bilingual abilities, 8:30am–4:30pm hours, and a 1.0 FTE with an assignment ending September 17, 2027. The team emphasizes wellness, professional growth, and work–life balance.

Qualifications

• Registered Nurse with a current Certificate of Registration from the College of Nurses of Ontario.
• Two years of related professional nursing experience.
• Palliative care experience including advance care planning and end-of-life care planning.
• Experience linking with community health agencies, needs assessment, care coordination or discharge planning.

Responsibilities

• Conduct comprehensive patient assessments using a validated palliative tool; address clinical, physical and psychosocial needs.
• Assess Palliative Performance Scale and eligibility for palliative and other services.
• Assess eligibility for hospice care and admission to palliative care units.
• Develop individualized care and service plans aligned with patient and caregiver priorities.
• Coordinate referrals to hospice and palliative care units and community resources.
