Job summary

Ontario Health atHome Champlain seeks a Bilingual Float Care Coordinator to assess patient needs, develop care plans, and coordinate access to home and community services. In this in-office role, you’ll work with a multidisciplinary team to support patients and caregivers from Montfort Hospital.

Responsibilities include comprehensive assessments, goal setting with patients, arranging services, and documenting care plans while ensuring equity and timely referrals.

Qualifications

• Registered with the appropriate Ontario college for the indicated profession.
• Two years of related professional experience preferred.
• Bilingual English and French required.
• Valid driver’s license and access to a vehicle.
• Vulnerable Sector Check completed within the last six months.

Responsibilities

• Completes comprehensive patient assessments using validated assessment tool, including clinical, physical and psychosocial needs, and caregiver needs.
• Assesses patient and caregiver understanding of diagnosis, treatment, resources, illness adjustment, and coping.
• Helps patient and caregiver set appropriate goals (individual, joint, short or long term).
• Collaborates with patient, caregiver and care team to develop an individualized care plan reflecting priorities and desired outcomes.
• Coordinates the delivery of home and community care services to patients and caregivers.
• Coordinates vendor and resource utilization involving medical equipment, supplies and services.
• Facilitates understanding among multidisciplinary healthcare team to maximize outcomes.
• Initiates and authorizes implementation of the care plan through contracted providers.
• Provides regular reports to physicians, service providers, patients, caregivers and others as required.
• Case documentation per Ontario Health atHome Champlain policies.
