Job summary

Amplify Care is seeking a Bilingual Change Management Specialist to lead adoption of digital health tools across community clinicians. You will apply structured change methods, build partner relationships, and integrate change activities into Amplify Care programs.

You will support EMR usage, align activities with health practitioners, and coach teams through transitions, with travel within the Eastern Provinces as needed. This project-based role emphasizes collaboration and impact.

Qualifications

• Must be bilingual in English and French.
• Minimum 3 years in a digital health or health care capacity.
• Post-secondary education in healthcare, IT, or information management.
• Experience with enabling technologies and EMRs.
• Strong communication and interpersonal skills.
• Travel within Eastern Provinces may be required.

Responsibilities

• Apply a structured change management approach for the people side of change.
• Be involved in the Change Management functional team projects and support priority work.
• Provide quality improvement supports to primary care clinicians and organizations.
• Ensure alignment of change management activities with health practitioners to smooth adoption.
• Incorporate adoption and change strategies based on situational awareness.
• Conduct implementation activities, document lessons learned and present findings.
• Be an active coach to clinicians and partners.
• Create workflows and manage measurement systems to track adoption and proficiency.
• Identify resistance and performance gaps and develop corrective actions.
• Provide reinforcement mechanisms and celebrate successes.
