Zusammenfassung

Ontario Health atHome is seeking a bilingual Care Coordinator to assess patient needs, coordinate home and community care, and connect with services across offices and facilities. You will work with patients, families, and providers to ensure accessible, patient-centered care.

The role requires English/French fluency, professional registration in nursing, OT, PT, SW, speech-language pathology or dietetics, and a valid driver’s license.

Qualifikationen

• Registered professional required in respective field and Ontario college registration
• Two years of related community-based experience preferred
• Bilingual English/French required or strongly preferred
• COVID-19 vaccination status proof required
• Valid driver’s license and access to a vehicle required

Aufgaben

• Assess patient care needs and eligibility and develop care plans
• Coordinate home and community care services and equipment
• Refer patients to community resources and programs
• Provide regular reports to physicians and caregivers
• Engage with patients, families and other healthcare partners
• Document case notes in Ontario Health atHome policies

Kenntnisse

Fluency in English and French

Verbal and written communication

Valid driver’s license

Microsoft Office

Experience in community health/care

Ausbildung

RN (BScN or Diploma) registered with Ontario College of Nurses

Physiotherapist registered with Ontario College

Occupational Therapist registered with Ontario College

Social Worker registered with Ontario College

Speech-Language Pathologist registered with CAOT/OSL

Dietitian registered with College of Dietitians of Ontario
