Ontario Health atHome Champlain is hiring a Bilingual Care Coordinator to assess patient needs, develop care plans, and arrange home and community services. You will work with patients, caregivers and a multi-disciplinary team to deliver patient-centered care from an intake setting.

The role emphasizes strong English/French communication, coordination with physicians and care providers, and adherence to organizational guidelines. Flexible in-office, community or facility-based work.

Qualifications

• Registered professional in the applicable Ontario college.
• Ability to assess diverse patient needs and coordinate care remotely or in person.
• Fluency in English and French required.

Responsibilities

• Manage a caseload of diverse patients with health concerns.
• Develop and update individualized care plans over the phone.
• Coordinate home and community care services with providers.
• Facilitate communication among multidisciplinary team members.
• Document case notes per Ontario Health atHome Champlain policies.
