Job summary

AgeCare Ltd. is seeking a Licensed Practical Nurse (LPN) to join our team in a resident-centered, compassionate setting. Reporting to the Director of Care, you will deliver goal-directed nursing care, conduct resident assessments, and coordinate care with physicians and the care team.

You’ll administer medications per policy, manage transitions, and support infection prevention, safety initiatives, and quality improvements. A relocation bonus up to $10,000 is available for eligible candidates.

Qualifications

• Active LPN license in the applicable province.
• Experience in senior or long-term care preferred.
• CPR and First Aid certification current.
• Strong communication and care coordination skills.
• Proficient with electronic health records and MS Office.

Responsibilities

• Collaborate with the care team using a person-centred approach.
• Maintain and update resident assessments, care plans, and documentation.
• Implement and adjust care plans as needed.
• Participate in care conferences and scheduling.
• Provide direct nursing care and administer medications per policy.
