Job summary

Radiant Care Tabor Manor is seeking an Assistant Clinical Quality Coordinator to assist with the ongoing operations of the RAI program, audits, and education. The role supports data accuracy, timely submissions, and quality improvement initiatives within a compassionate long-term care environment.

The successful candidate will have RN or RPN credentials, an active RAI certificate, and strong analytical skills to drive resident-centered outcomes and maintain compliance with relevant standards.

Qualifications

  • Must be Registered Practical Nurse in good standing with the Ontario College of Nurses.
  • Completion of a program of nursing (i.e., RN or RPN)
  • Current RAI certificate
  • Certified Mask Fit Tester or willingness to obtain certification
  • Strong knowledge of RAI-MDS processes and reporting requirements
  • 1-3 years' experience in long-term care nursing
  • Must have a current RAI Certification
  • Knowledge of quality indicators and clinical best practices
  • Experience conducting audits and reviewing clinical documentation
  • Strong analytical and organizational skills
  • Ability to interpret data and identify quality improvement opportunities
  • Excellent communication and education skills
  • Ability to work independently and collaboratively with interdisciplinary teams

Responsibilities

  • Assist with planning, coordination, implementation, and ongoing operations of the RAI program.
  • Coordinate and support timely completion, coding, validation, and submission of MDS assessments, RAPs, care plans, and other RAI documentation.
  • Maintain and monitor the resident assessment schedule to ensure assessments are completed accurately and within required timelines.
  • Support RAI users in developing processes that promote data accuracy, efficient data collection, assessment completion, and effective use of RAI data.
  • Ensure compliance with RAI requirements, organizational policies, applicable legislation and regulations, professional standards, interRAI requirements, and CIHI/IAR submission standards.
  • Analyze RAI data, quality indicators, and assessment outcomes to identify trends, risks, and opportunities for quality improvement.
  • Collaborate with the Clinical Quality Coordinator, registered staff, and nursing leadership to promote assessment accuracy, effective care planning, and quality resident outcomes.
  • Provide RAI education, training, coaching, and on-site support to RAI users, including computer application training, coding, documentation requirements, learning plans, and resource development.
  • Observe and evaluate staff application of RAI knowledge and skills and provide feedback to accountable managers regarding competency and performance, as appropriate.
  • Conduct and assist with RAI and nursing audits; identify quality assurance issues and provide education, corrective direction, and follow-up to address identified gaps.
  • Assist with the development and implementation of quality improvement initiatives and action plans based on audit findings, RAI outcomes, and identified trends.
  • Assist with overseeing the Nursing Restorative Care Program in collaboration with the interdisciplinary team.
  • Promote Radiant Care's Montessori Philosophy of Care and support resident-centred care, safety, dignity, and quality of life.
  • Maintain resident and staff confidentiality and contribute to due diligence by ensuring ethical, legal, and professional requirements are met.
  • Maintain current knowledge of and comply with applicable legislation, including long-term care legislation, AODA, Residents' Rights, Occupational Health and Safety requirements, and other applicable standards and regulations.
  • Understand and comply with Radiant Care policies and procedures and the Collective Agreement, where applicable.
  • Participate in annual goal setting and contribute to the achievement of objectives aligned with Radiant Care's Strategic Plan.
  • Complete all mandatory training and professional development requirements.
  • Perform other duties as assigned by the Supervisor.

Job Description
Position Summary

Reporting to the Clinical Quality Coordinator, the Assistant Clinical Quality Coordinator assists with the ongoing operations of the Resident Assessment Instrument (RAI) program, as well as quality improvement, audits and education.
