Job Description:

The overall function of the Assessor/Coordinator (Client Care Coordinator) position is to provide case management services including assessment, planning, monitoring, and evaluation. The client/patient access services primarily handle single entry access and ongoing coordination for various community-based services within SHA (Saskatoon).

Qualifications:

• Bachelor of Science in Nursing (BScN) or acceptable alternatives such as Bachelor of Nutrition, Bachelor of Occupational Therapy, Bachelor of Social Work, Bachelor of Physical Therapy.
• Knowledge of SHA, community resources, and applicable programming.
• Preference for HSAS members as per the SAHO/HSAS Collective Agreement.
• Ability to travel within the region and provincially.
• Registered or eligible for registration with the appropriate licensing body.
