Job Description: The overall function of the Assessor/Coordinator (Client Care Coordinator) position is to provide case management services which includes assessment, planning, monitoring and evaluation. The client/patient access services is primarily responsible for a single entry access and ongoing coordinator for a variety of community based services within SHA (Saskatoon).
Human Resources Exemption: No
Education: Bachelor of Science in Nursing (BScN). Other acceptable degrees include Bachelor of Occupational Therapy, Bachelor of Social Work, Bachelor of Physical Therapy, Bachelor of Nutrition.
Knowledge and Abilities: Knowledge in the SHA, community resources, and applicable programming.
