Job Description : The overall function of the Assessor / Coordinator (client care coordinator) position is to provide case management services, including assessment, planning, monitoring, and evaluation. The client/patient access services primarily handle single entry access and serve as ongoing coordinators for various community-based services within SHA (Saskatoon).
