Job summary

Fraser Health Authority seeks a Registered Nurse to provide direct outpatient nursing care, assess and plan care, and support patients across diverse settings. The role emphasizes patient-centered care, teamwork with physicians and care coordinators, and ongoing professional development.

You will contribute to discharge planning, collaborate with care teams, and participate in quality improvement initiatives, while adhering to Fraser Health’s standards and values.

Qualifications

• Graduation from an approved School of Nursing.
• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Two years recent clinical experience in the designated clinical area and completion of post graduate course work and/or certification applicable to the designated clinical area, or an equivalent combination of education, training and experience.

Responsibilities

• Assess patients and collect information from a variety of sources using observation, communication, data review and physical assessment.
• Observe patient acuity and intervene to provide and manage care as necessary.
• Assist physicians during procedures and manage related equipment and supplies.
• Collaborate with patients and families in developing and modifying care plans.
• Coordinate nursing care by prioritizing tasks and delegating to support staff.
• Collaborate with interprofessional care team to adjust care plans as needed.
