Job summary

Fraser Health in British Columbia is recruiting a registered nurse for outpatient care in Surrey. You will provide direct, individualized nursing care to outpatients with varying illness, through minor procedures and endoscopy, assess needs and deliver discharge teaching.

Collaborate with an inter-professional team to plan and implement care, monitor patient acuity, assist with procedures, coordinate referrals and discharge planning, and document care and outcomes.

Qualifications

• Graduation from an approved School of Nursing.
• Current RN registration with the BC College of Nurses and Midwives (BCCNM).
• Two years of recent clinical experience in the designated area, or equivalent education/experience.
• Post-graduate course work or certification relevant to the area.

Responsibilities

• Assess patients and collect information from multiple sources.
• Monitor patient acuity and intervene as needed.
• Assist physicians during procedures and manage equipment.
• Develop and modify care plans with the interprofessional team.
• Coordinate discharge planning and referrals with the care team.
• Document assessments, interventions and outcomes.
• Participate in quality improvement activities.
