Job summary

NHS Greater Glasgow and Clyde is seeking an Advanced Frailty Practitioner to join the Home First Response Service (HFRS). A live NMC UK pin is required.

Band 7, based at Queen Elizabeth University Hospital, with a permanent or 6-month fixed-term contract and full-time hours; a variety of patterns considered at interview. The role focuses on comprehensive frailty assessments, clinical decision-making, safe discharge, and coordination with acute, community and social care partners to prevent

Qualifications

• Applicants should be registered healthcare professionals with substantial post-registration experience, ideally in older people’s services, urgent care or community settings.
• Postgraduate qualifications in advanced clinical practice, frailty, or clinical assessment are required.
• Band 7 role within NHS Scotland with focus on frailty and home-first models.

Responsibilities

• Undertaking comprehensive frailty assessments for older adults.
• Supporting clinical decision-making and development of management plans.
• Facilitating safe discharge and admission avoidance where appropriate.
• Working collaboratively with acute, community and social care partners.
• Coordinating referrals and onward care arrangements.
• Providing leadership and expert clinical advice within the multidisciplinary team.
