Job summary

St. Joseph's Health System (SJHS) and SJH St. Joseph's Healthcare Hamilton are seeking a Registered Social Worker to provide psychosocial assessment, counselling, case management and discharge planning across designated sites in Hamilton.

The role supports patient and family education, care coordination and collaboration with the care team to optimize recovery and quality of life. The successful candidate will demonstrate strong interpersonal and communication skills, be able to work in a team

Qualifications

• Bachelor of Social Work (clinical) required
• Masters of Social Work (clinical) preferred
• Registration with OCWSSW required
• Membership with OASW preferred
• 2 years acute care hospital social work experience preferred
• Able to work in team and independently
• Excellent communication and interpersonal skills
• Strong organizational and time management abilities
• Demonstrated computer skills
• Good attendance record and resume required
• Knowledge of seniors care and family support preferred
• Gentle Persuasive Approach training preferred

Responsibilities

• Maintain knowledge of relevant legislation (Mental Health Act, Health Care Consent Act, Substitute Decisions Act, PHIPA, etc.)
• Provide psychosocial assessments, counselling, case management, discharge planning and care coordination
• Document interventions adhering to standards and guidelines
• Conduct comprehensive psychosocial assessments from a systems perspective
• Identify and intervene with high-risk social situations
• Provide theory-based counselling and crisis intervention
• Coordinate complex discharges and program-specific case management
• Lead discharge planning and family meetings as needed
• Mentor new social work staff and other disciplines
• Meet requirements for OCSW re-certification through continuing education
• Comply with workload measurement and data collection
• Perform other duties as assigned
