Qualifications

• Bachelor’s Degree in Social Work required.
• Registered with the Ontario College of Social Workers and Social Service Workers required.
• Membership with the Ontario association of Social Workers preferred.
• Minimum one year recent related experience required.
• Gentle Persuasive Approach training preferred.
• Experience in Home or Community care settings preferred.

Responsibilities

• Provide psychosocial assessment, counselling/psychotherapy and case management.
• Coordinate discharge planning and service coordination to aid recovery and quality of life.
• Conduct comprehensive psychosocial assessments from a systems perspective.
• Mentor program staff and participate in quality improvement activities.
• Maintain patient-related documentation according to standards.
