Job summary

Oak Valley Health is seeking a Dietitian to join the interprofessional team at Markham Stouffville Hospital and the RCC site. You will perform nutritional assessment, develop and implement care plans, and evaluate patient progress for acute and subacute adult populations, using evidence-based practices.

The role requires Canadian Dietitians registration, 2+ years in acute care, and strong leadership, communication, and teamwork skills. Travel between sites and EMR proficiency are expected.

Qualifications

• Current membership in the College of Dietitians of Ontario, in good standing; Dietitians of Canada membership preferred; CNSC designation asset
• A minimum of 2 years clinical experience in nutrition care support of adult patients in acute care
• Strong skills within paediatric and NIC population required
• Commitment to ongoing professional development and evidence-based practice
• Excellent assessment, organizational, and interpersonal skills
• Excellent verbal and written communication skills
• Competency with EMR charting systems, CBORD, and related computer skills
• Ability to work independently and as part of an interprofessional team
• Access to own transportation to travel between sites
• Good attendance and performance records with ability to maintain standards
• Commitment to a healthy and safe workplace with adherence to policies and training

Responsibilities

• Nutritional assessment and care plan development for acute and subacute adult patients
• Implementation and evaluation of nutrition care plans
• Coordinate nutrition services to bridge hospital to home transitions
• Contribute to quality improvement initiatives
• Lead and collaborate with interprofessional team to deliver optimal care
