Job summary

Humber River Health in Toronto is seeking a Registered Dietitian to join the Primary Care Inter-professional team. The RD will assess, plan, implement, and evaluate nutrition care for patients referred to the service, focusing on chronic disease management and prevention in a primary care setting.

Qualifications

• Bachelor's or Master's in Food and Nutrition required.
• CDO registration in good standing required.
• Completion of accredited Dietetic Internship and Dietitians of Canada membership or eligibility.
• 2 years of clinical experience preferred.
• Diabetic Educator certification preferred.
• Experience teaching group classes online and in-person.

Responsibilities

• Collaborates with physicians, residents, nurses, pharmacist and social worker to integrate nutrition in chronic disease care.
• Develops patient-focused nutrition care plans and provides counseling.
• Assesses nutritional status and progress; supports behavior change toward healthier eating.
• Develops and maintains programs, workshops, and educational resources.
• Documents care in the electronic health record and shares outcomes with the care team.
• Coordinates diagnostic tests and referrals as per RD scope; trains students/volunteers.
• Performs other duties and participates in Family Health Team rounds.
