Taking part in community health is rewarding and meaningful. LifeLong Medical Care is looking for a full-time Physician, Nurse Practitioner, or Physician Assistant committed to providing high-quality outpatient primary care at our Brookside San Pablo Health Center.

As a Physician/FNP/PA at our Health Center, you'll work alongside a multidisciplinary team of healthcare professionals to provide comprehensive care to our patients. Our Health Center utilizes a collaborative team model, integrating case management and behavioral health services to address medical and psychosocial factors impacting our patients' overall health.

This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA). Employees must remain in good standing with UAPD, as defined in the CBA.

About us:

As a Federally Qualified Health Center in Northern California with over 45 years of service, LifeLong serves over 66,000 patients in underserved regions of the East Bay Area. We provide medical, dental, behavioral health, and school-based services at over 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. We innovate and respond to social and medical needs, offering services like homeless respite programs and free COVID testing. Benefits:

Compensation: $197,000-$218,000/year for MD/DO and $123,000-$146,000/year for APP, depending on experience. Includes sign-on bonus and benefits: medical, dental, vision, FSA, dependent and partner coverage, 403(b), and loan repayment programs. Our site qualifies for federal and state loan repayment (HPSA score 21). Rates vary yearly based on funding and applicants.

Responsibilities
  1. Provides preventive, urgent, and primary care health services
  • Performs clinical assessments, orders diagnostics, and plans care
  • Provides education and counseling to patients and families
  • Plans follow-up care, including outside consultations
  • Maintains client records for reporting, billing, and quality improvement
  • Consults with supervising physician as needed
  • Supports triage functions by phone and walk-in
    • Recognizes urgent situations requiring physician attention
    • Provides interim or definitive care
  • Functions in various settings including clinics, nursing facilities, housing sites, and homes
  • Participates in clinical team activities
    • Attends team meetings
    • Provides specialty care, case management, and program participation (e.g., HIV, women’s health, diabetes)
    • Coordinates care with team members
    • Advocates for patients and liaises with outside agencies
    • Engages in quality improvement initiatives
  • Adheres to the LifeLong Clinicians Association Code of Conduct
  • Participates in internal quality improvement efforts
  • Performs other duties as assigned
  • Qualifications
    • Interest in working with diverse patient populations in a community setting
    • Effective communication skills with diverse backgrounds; Spanish is a plus
    • Ability to work independently with minimal supervision
    • Knowledge of Northern Alameda County resources is desirable
    • Valid California driver’s license, auto, and insurance
    • Experience with electronic health records, especially Epic, preferred

    Education and Experience

    • Current California license
    • Background in community health clinics preferred
    • Experience in team-based care models preferred
