Qualificações

Purpose and Scope

The professional registered nurse Outpatient RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegation of tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities

• Perform all essential functions under supervision and guidance from the Clinical Educator, Preceptor, or collaborating RNs.
• Collect and analyze patient data pre-, during-, and post-hemodialysis, document findings, and adjust treatment plans accordingly, notifying appropriate staff as needed.
• Assess and document patient and family educational needs, providing ongoing education.
• Coordinate and deliver safe, effective hemodialysis therapy in collaboration with care staff.
• Administer prescribed medications and document their justification and effectiveness.
• Initiate or assist with emergency responses.
• Serve as a resource, mentor, and role model for staff, demonstrating appropriate behavior and work habits.
• Ensure proper laboratory procedures and reschedule missed collections.
• Update nursing assessments and care plans through collaboration with the interdisciplinary team.
• Educate patients about transplant and treatment options.
• Participate in quality improvement projects and may serve as a preceptor.
• Complete CAP requirements to maintain or advance certification.
• Perform other duties as assigned by the supervisor.

Physical Demands and Working Conditions

• Involves heavy lifting, patient movement, and assistance with ambulation, with equipment and coworker assistance as needed.
• Requires frequent standing, bending, and possibly moving equipment up to 200 lbs.
• Work environment typical of healthcare facilities, with potential exposure to infectious materials.
• Includes desk and computer work, patient interactions, and possible travel.

Supervision

• Oversees Patient Care Technicians, LVNs, LPNs, or RNs as a Team Leader or Nurse in Charge after meeting training and experience requirements.

Education and Licensure

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/BLS certification.
• Meet state practice requirements.

Experience and Skills

• Minimum 2 years of Nephrology Nursing experience; entry-level RNs considered.
