Job summary

Everyday Independence is hiring a Youth Lead Practitioner to support adolescents and young adults with disabilities. You will lead the interdisciplinary team, plan NDIS therapy packages and act as main contact for participants and families. A relevant credential and registration are required.

Minimum 2 years' experience with youth 9-25, strong communication, flexibility and drive to grow professionally. Driver's license, checks and patient-centered approach are essential.

Qualifications

• Relevant tertiary qualification and registration.
• Youth Worker path includes YWA membership.
• Social Worker path includes AASW membership.
• Minimum 2 years experience working with youth aged 9-25.

Responsibilities

• Led the interdisciplinary youth team planning a young person’s NDIS therapy package.
• Supported youth to achieve educational, personal and social goals.
• Delivered therapy across homes, schools, playgrounds and community settings.
• Held future-focused conversations with participants and supports.
• Empowered families to develop routines and habits.
• Built strong relationships with participants and carers/family.
• Documented and billed each session in the online system.
• Joined weekly team meetings with the Hub and mentors.
