Job summary

Everyday Independence in Hallam, Melbourne, is seeking a Youth Lead Practitioner to support adolescents and young adults with disabilities. You will lead a youth team, plan NDIS-funded therapy packages and work across homes, schools, and community centres to help participants achieve their goals.

The role requires a relevant qualification and registration, 2+ years of youth work experience, a driver’s license, and checks such as NDIS Worker Screening and Working with Children’s Check.

Qualifications

• Minimum 2 years experience working with youth aged 9-25
• A relevant tertiary qualification and registration is required
• A valid Australian driver’s license
• NDIS Workers Screening Check and Working with Children’s Check

Responsibilities

• Lead the interdisciplinary youth team planning a young person’s NDIS funded therapy package and act as main contact for participants and families
• Use assessment and intervention skills to help youth achieve educational, personal, social and working-life goals
• Work across diverse settings including homes, schools, playgrounds, workplaces and community centres
• Document and bill for each session through the online participant management system
• Attend weekly team meetings and collaborate with Team Leader and Therapy Mentor for support
