Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults with disability, aged 9-25, across multiple settings. You will coordinate an interdisciplinary team to plan and manage NDIS-funded therapy packages, and work closely with families and supports to achieve education, personal, social and work goals.

Qualifications

• Minimum 2 years experience working with youth aged 9-25
• Relevant tertiary qualification and registration
• Willingness to work toward billable targets
• Valid Australian driver’s license
• NDIS Worker Screening Check
• Working with Children’s Check

Responsibilities

• Lead the interdisciplinary youth team planning NDIS funded therapy packages
• Support youth to achieve educational, personal, social and work goals
• Deliver therapy in diverse settings (homes, schools, community sites)
• Conduct future-focused conversations with participants and supports
• Document and bill for sessions through online system
• Engage with Team Leader and Therapy Mentor for guidance
