Job summary

Everyday Independence is seeking a Youth Lead Practitioner in Melton to support adolescents and young adults (10–24) with disability. You will join a team delivering therapy across homes, schools and community settings with a focus on independence and goal attainment.

The role offers flexible working options and a strong mentoring culture, with opportunities for professional growth and structured supervision. A valid checks and registrations are required.

Qualifications

• Minimum 12 months experience working with youth aged 10-24
• Willingness to work to billable targets
• NDIS and allied health context experience preferred
• Registration/endorsement as required by chosen discipline
• Commitment to growing professional skills and mentorship
• Ability to work in diverse settings (home/school/community)

Responsibilities

• Lead the interdisciplinary team planning a young person’s NDIS funded therapy package
• Support youth to achieve educational, personal, social and work goals
• Deliver therapy across diverse settings (home, school, community) within a social model
• Maintain client relationships with participants and their supports
• Document and bill sessions through the online participant management system
• Attend weekly hub team meetings and engage in clinical mentorship
