Job summary

Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults (9-25) with disability, across multiple locations. You will lead an interdisciplinary youth team, plan NDIS therapy packages, and work in homes, schools, and community settings.

Ideal candidates bring 2+ years in youth work and/or social work, with registration and a commitment to growth. Drive to meet billable targets and engage families is essential.

Qualifications

• Minimum 2 years experience working with youth aged 9-25
• Positive and flexible approach to work
• Strong communication skills and be a team player
• Commitment to personal and professional growth
• Demonstrated experience working with people with a disability
• Willingness to work to billable targets
• A valid Australian driver’s license
• NDIS Workers Screening Check
• Working with Children’s Check

Responsibilities

• Led the interdisciplinary youth team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
• Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
• Delivered therapy in diverse settings including homes, schools, playgrounds, workplaces and community centres
• Held future-focused conversations with participants and supports
• Empowered participants and families to develop habits and routines
• Built meaningful relationships with participants and supports
• Documented and billed each session in the online participant management system
• Attended weekly team meetings to share impact and collaborate
• Met with Team Leader and Therapy Mentor for clinical guidance
