Melbourne City Mission (MCM) is seeking a Case Manager/Youth Coach for a 6-month, full-time max-term role in Western Metro Melbourne. You will provide outreach and case management to young people at risk of homelessness, helping them set goals and stabilise or secure housing.

The role requires collaboration with partners, delivery of group activities, and robust reporting to support client outcomes. A supportive, inclusive workplace with development opportunities awaits.

Formación

• Involves outreach and comprehensive case management for young people and families.
• Experience with early intervention and youth homelessness sector is essential.
• Ability to work with education and welfare sector partners to achieve client outcomes.

Responsabilidades

• Provide outreach and case management support for at-risk youth and families.
• Apply Detour coaching framework in service delivery.
• Plan and co-facilitate group work initiatives.
• Complete regular reporting, data collection, and record keeping.
• Promote the program both internally and externally.
