Lives Lived Well in Lithgow is seeking an experienced Youth AOD Counsellor & Case Manager to support 12-24 year olds through counselling, case management and community-based services. You’ll help young people build resilience and plan pathways forward within the Dianella Youth Program.

You’ll collaborate with families, schools and partners, deliver outreach across Lithgow, and advocate for positive change in the community. This role is full-time and based on-site.

Competenze

• Experience supporting young people within health, community services or AOD settings.
• Strong communication and relationship-building skills.
• Trauma-informed, person-centred and recovery-oriented practice knowledge.

Mansioni

• Provide counselling and therapeutic support to young people.
• Complete assessments and develop individual support plans.
• Support young people to identify strengths, goals and pathways forward.
• Deliver outreach support across Lithgow and surrounding communities.
• Facilitate youth-focused groups and education programs.
• Work with families, schools and community partners.
• Maintain client records and contribute to service improvement.
• Connect young people to additional supports and services when required.
