Job summary

Gateway Health’s Head to Health team in Wodonga is seeking an experienced Wellness Practitioner to provide comprehensive mental health assessments, develop tailored treatment plans and deliver evidence-based brief interventions, single-session therapy, counselling and group programs.

You will coordinate care for people with complex needs, connect clients with the right services, and work with families, carers and community partners to support lasting wellbeing.

Qualifications

• A relevant tertiary qualification in Health, Social Work, Welfare, Community Services, Counselling or related discipline.
• Minimum of 2 years’ experience in the Mental Health sector.
• Demonstrated experience delivering a variety of evidence-based approaches.
• Strong interpersonal, problem-solving, and negotiation skills.
• Excellent time management and organisational skills.
• Well-developed communication skills.

Responsibilities

• Conduct comprehensive mental health assessments and develop tailored treatment plans.
• Deliver evidence-based brief interventions, single-session therapy, counselling, and group programs.
• Coordinate care for people with complex needs and connect clients with the right services.
• Collaborate with families, carers and community partners to support wellbeing outcomes.
• Role based in Wodonga with occasional travel.

About the role

Are you an experienced mental health professional who is passionate about helping people move from distress towards recovery? Join Gateway Health’s Head to Health team and make a genuine difference by delivering responsive, person-centred support across Wodonga and surrounding communities.

As a Wellness Practitioner, you will work within a collaborative, recovery-oriented team to provide comprehensive mental health assessments, develop tailored treatment plans and deliver evidence-based brief interventions, single-session therapy, counselling and group programs. You will also coordinate care for people with complex needs, connect clients with the right services and work alongside families, carers and community partners to support meaningful and lasting wellbeing outcomes.

This role is based in Wodonga, with occasional travel to other sites, and is classified at SACS Level 5 under the Community Health Centre Stand Alone Services Social and Community Service Employees Multi Enterprise Agreement 2022.

Skills & Experience

To thrive in this role, you'll bring:
