Job summary

BlueCare Caloundra Aged Care Facility, part of UnitingCare, seeks a Wellness Partner to deliver engaging wellness activities for residents. The role emphasizes inclusivity, person-centred care and collaboration with care staff, volunteers and families to promote independence and quality of life.

Applicants should have a Certificate IV in Leisure and Health or equivalent, plus experience guiding group and one-to-one activities in aged care or community settings.

Qualifications

• Certificate IV in Leisure and Health or equivalent.
• Experience delivering group and 1:1 activity in aged care or community settings.
• Practical experience in creative, social, cognitive and physical activity facilitation.
• Understanding of dementia care, person-centred practice and cultural safety.
• Knowledge of infection control, manual handling practices and Strengthened Aged Care Quality Standards.
• Exposure to person-centred care principles and needs of older adults.
• Experience working as part of a multidisciplinary team.

Responsibilities

• Facilitate inclusive group and 1:1 wellness activities for residents’ physical, emotional, social and cognitive wellbeing.
• Build relationships with residents to support independence, dignity and participation.
• Collaborate with care staff, families and volunteers to align programs with resident needs.
• Guide and engage volunteers to contribute effectively to resident wellbeing.
• Record lifestyle assessments and observe changes in wellbeing per escalation processes.
• Prepare activity spaces and materials and maintain safe, hazard-free environments.
