Job summary

ermha365, part of Meridia, is recruiting for a Wellbeing Mentor (Lived Living Experience or Non-Lived Experience) in Dandenong South, VIC. The role provides assertive community outreach and coordination of CPS-funded psychosocial rehabilitation services to people with significant mental illness, using a strengths-based, trauma-informed approach.

You will work with participants and families in home and community settings, build capacity, and manage risk with thorough case notes.

Qualifications

• NDIS Work Screening, Working With Children’s Check, work rights compliance required.

Responsibilities

• Provide case management and assertive outreach to support a portfolio of participants and their families in their own home and community
• Provide short term assistance to enable participants to work towards their goals
• Assist to build capacity and work towards outcomes identified by the participant
• Develop and maintain professional relationships with participants and stakeholders
• Provide accurate and timely case notes including risk management and recovery plans
• Extensive outreach across a catchment area of over 10 LGAs in the South East Melbourne region; personal vehicle required
• Hybrid working model with flexible service delivery between 8am-8pm Monday-Friday based on consumer needs
