Qualifications

• Minimum 3-5 years of experience in a relevant position.
• Eligibility for registration with AHPRA, AASW, or PACFA.
• Extensive knowledge and experience in Strength-based Case Management.

Responsibilities

• Work with veterans and their families in homelessness and financial hardship programs.
• Assess clients and prepare case management plans.
• Conduct home and hospital visits to provide emotional support.
