Job summary

Empower Community Health Care Services is urgently recruiting five Mental Health & Disability Support Workers to assist NDIS participants in the Teralba and Maitland areas. You will provide person-centred supports in homes and the community, focusing on mental health and disability needs.

Applicants should have at least one year of relevant experience, a valid drivers licence, and NDIS checks. The role is casual with flexible hours and requires reliable transport and strong documentation skills.

Qualifications

• Minimum 1 year experience in mental health support.
• Minimum 1 year experience in disability support.
• Current Driver's Licence.
• Reliable vehicle with comprehensive insurance.
• NDIS Orientation Module completed.
• NDIS Worker Screening Check completed.
• Two professional work references.

Responsibilities

• Mental health and psychosocial support in participants' homes and community.
• Daily living and personal care tasks.
• Community access and social engagement.
• Medication prompting (where applicable).
• Behaviour support plan implementation.
• Routine-based supports.
• Transport to appointments and activities.
• Work independently and follow participant plans.

About you
