APM is seeking a Clinical Aged Care Assessor to identify needs of older adults and connect them with services enabling independent living and improved quality of life. Daily travel across the service area is required to meet client needs.

The role involves high-quality assessments, service referrals, and promoting functionality, with collaboration across families, carers and a multidisciplinary team. Mandatory pre-employment checks apply.

Qualifications

• At least 1 year working with older Australians, including client assessments and/or service delivery.
• Proficiency with client management systems.
• Proficiency in Word, Excel and Outlook.

Responsibilities

• Conduct High-Quality Aged Care Assessments to identify needs of older adults.
• Facilitate referrals to essential services supporting independent living.
• Promote retention and maximization of function and daily living capabilities.
• Engage with families and carers as key stakeholders.
• Ensure assessments comply with Department of Health contractual requirements.
