Youturn Limited invites applications for a Case Officer in the Family & Youth Services team. You will work with children, young people and families facing complex needs, delivering intensive case management, assessment, advocacy and coordinated intervention across schools and community settings.

The role involves collaboration with stakeholders, practical and therapeutic support, risk assessments and detailed case planning to improve wellbeing and sustainable outcomes for clients.

Qualifications

• Diploma or higher qualification in Community Services, Human Services, Mental Health, Youth Work, Counselling, Social Work, Family Intervention or a related field.
• Blue Card Working With Children Check (WWCC) required.
• Open Driver Licence and ability to obtain and maintain screening and suitability checks.

Responsabilités

• Deliver client-centred case management services.
• Conduct comprehensive assessments, safety planning and risk assessments.
• Develop, implement and review individualised case plans.
• Support families experiencing trauma, conflict or education disengagement.
• Coordinate referrals across government and community services.
• Provide advocacy, practical support, life skills and parenting support.
• Build partnerships with schools, health providers and community agencies.
• Maintain accurate case notes and reports in line with funding and compliance.
• Participate in collaborative case reviews and service improvements.
