Job summary

Calvary Health Care Kogarah is seeking a Transitional Nurse Practitioner to provide high-level clinical expertise within the Community Palliative Care Team across RACF, HiTH and ambulatory clinics.

You will be a registered NMBA RN (Division 1) with at least 5 years post-registration experience, including 3 years in palliative/end-of-life care, and you will pursue a Nurse Practitioner master's degree or be eligible for entry. The role emphasizes leadership, education and quality improvement.

Qualifications

• Current NMBA registration as Registered Nurse (Division 1).
• Minimum 5 years' full-time equivalent post-registration experience.
• At least 3 years' experience in Palliative Care/End of Life care.
• Postgraduate qualification in Palliative Care or related field.
• Enrolled in NMBA approved NP master's or eligible.

Responsibilities

• Rotate between RACF, Palliative Care HiTH and CPCT clinics.
• Provide high-level assessment, interpret diagnostics, implement treatments and referrals.
• Lead changes in clinical practice and develop direction with evidence-based research.

About the role

The Transitional Nurse Practitioner (TNP) provides readily available access to high level clinical expertise and support for the Calvary Palliative Care Service. The Transitional Nurse Practitioner’s extended practice role includes, comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex, and changing situations within the Palliative Care Service.

The TNP is an integral member of the Community Palliative Care Team (CPCT), and this position will rotate between RACF, Palliative Care HiTH and supporting clinical practice within CPCT including running ambulatory care clinics for CPCT patients

About you

• Ability to work within the Mission and Values of Calvary Health Care Kogarah
• Transitional Nurse Practitioner: Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) and a minimum of 5 years' full time equivalent post registration experience and at least 3 years' full time equivalent experience in Palliative Care, End of Life and post graduate qualification in Palliative Care or area related to the clinical specialty. Transitional Nurse Practitioner: Enrolled in a Nursing and Midwifery Board of Australia (NMBA) approved Nurse Practitioner's master's degree or able to meet course entry requirements at the time of appointment.
• Demonstrated high level knowledge and advanced clinical nursing skills including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex, and changing situations.
• Demonstrated understanding of the palliative care delivery system and ability to lead and implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence-based research and ability to undertake clinical service planning
• Proven high level knowledge and expertise in complex client centred consultancy, practice development activities including mentoring, education, active participation in policy development and quality improvement
• Demonstrated advanced clinical leadership role, negotiation, liaison, interpersonal and problem-solving skills across the palliative care service that includes acute, sub-acute and aged care sectors
• Current NSW Class C Unrestricted Driver’s License and Working with Children Check
