Qualifications

• Tertiary degree in Social Work is required.
• Eligible for membership to the Australian Association of Social Workers (AASW).
• Vaccination as required by policy including Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B.

Responsibilities

• Provide an efficient and responsive social work service appropriate to older patients and their families/carers.
• Undertake case management including coordination of internal and brokered services.
• Conduct home visits and site visits across Gold Coast Health facilities.
