Job summary

Gold Coast Health is seeking a Social Worker for the Transitional Care Program on the Gold Coast. The role involves delivering a patient- and family-centered social work service, undertaking case management, and coordinating care across internal and brokered services. Home visits and site visits are part of the duties.

You will receive supervision from the Transition Care Program Senior Social Worker and develop clinical expertise through ongoing learning within the team.

Qualifications

• Tertiary degree in Social Work is required.
• Eligible for membership to the Australian Association of Social Workers (AASW).
• Vaccination as specified (MMR, Varicella, Pertussis, Hepatitis B) or be willing to obtain it.
• Driver Licence is required.
• Aged Care Check (cost covered by GCH).
• Criminal History Screening required (cost covered by GCH).

Responsibilities

• Provide an efficient and responsive social work service to patients and families/carers.
• Undertake case management including coordination of internal and brokered services.
• Conduct home visits and visits to other Gold Coast Health sites.
