Job summary

Gold Coast Health is seeking a Social Worker to join the Transitional Care Program (TCP) based at Southport, QLD. The role focuses on delivering an efficient social work service for patients and families, with responsibilities including case management and coordinating internal and brokered services.

You will perform home visits, receive supervision from the TCP Senior Social Worker, and contribute to ongoing learning within a supportive multidisciplinary team.

Qualifications

• Tertiary degree in Social Work and eligible for AASW membership.
• Vaccination status must be up to date.
• Driver Licence.
• Aged Care Check provided by employer.
• Criminal History Screening required.

Responsibilities

• Provide an efficient social work service for patients and families.
• Undertake case management and coordinate internal and brokered services.
• Perform home visits and visits to other Gold Coast Health sites.
• Receive clinical supervision from the Transition Care Program Senior Social Worker.
• Develop clinical knowledge through active learning within the team.
